Evaluations Dates - 2011

The evaluations will be held on Sunday, September 18, 2011 at Chic Murray Arena - Burnhamthorpe Community Centre.

If your child's last name starts with the letter A to J , please come for the 1 p.m. evaluation time.

If your child's last name starts with the letter K to Z, please come for the 2 p.m. evaluation time.
 
If you are new this year, please bring a copy of your child's birth certificate.  If you are paying for registration on that day, please bring cash, cheque payable to Applewood Hockey Association or credit card information.
